If you purchased potatoes from in Arizona, California, Florida, Iowa, Kansas, Massachusetts, Michigan, Minnesota, Nevada, New York, North Carolina, Tennessee, Vermont or Wisconsin from October 14, 2004 toApril 10, 2015, you are eligible for a settlement rebate.

Class Members who wish to file a claim online must complete this form no later than October 16, 2015 in order to be considered for benefits. All you have to do is fill out the form here and then wait for a rebate check in the mail.

There’s no proof of purchase required, so you can file a claim even if you haven’t saved your receipt. The rebate amount will depend on the number of people that file claims.
